To help us get to know you and ensure your first visit is as comfortable and seamless as possible, we kindly ask that you bring the following with you to your initial appointment:

• A Valid Photo Identification: This helps us confirm your identity.
• Your Dental Insurance Card(s) or Information: Please bring any forms your insurance company has provided.
• A List of Current Medications: Knowing your current medications helps us provide the safest and most effective care.

We understand that navigating dental insurance can sometimes feel a little tricky, and we’re here to help! Our friendly team works with many different insurance companies and will gladly file your dental claim on your behalf whenever possible. We currently accept the following popular plans:

• Met Life Traditional PPO
• Cigna PPO
• Guardian PPO and Traditional
• Horizon Blue Cross Blue Shield Traditional
• Aetna PPO
• Delta Premier and Advantage

If you don’t see your insurance listed, please don’t worry! We encourage you to give our office a call today to verify acceptance of your specific plan. We’re always happy to assist you in understanding your benefits.

We value your time and strive to provide efficient and timely care for all our patients. If, for any reason, you find that you need to reschedule or cancel your appointment, we kindly request at least 24 hours’ notice.
