Thanks for selecting our practice to look after your oral health care. As your new partner in dental health care, allow us to identify you correctly and help us make your transition from your previous oral health care provider as quick and smooth as possible.

To allow us to serve you better on your first appointment, be sure to bring:

  • Identification
  • Insurance cards or forms provided to you by your insurance company
  • A list of any current medications you may be taking
  • Any recent x-rays if they are available

Insurance

We deal with many different insurance companies and will be happy to file a dental claim, where applicable, with your insurer on your behalf. We currently accept Met Life Traditional PPO, Cigna PPO, Guardian PPO and Traditional, Horizon Blue Cross, Blue Shield Traditional, Aetna PPO, and Delta Premier and Advantage.

Cancellation

In case you may not be able to make your appointment, please provide us with 24 hours notice so we can schedule this appointment for another patient. If a patient doesn’t not show up for an appointment without notice, a fee may be charged.
